Updates to Incident Reporting

Per OSHA recommendations, we have added instructional text to 3 existing questions on our Encounter Case/Incident. This was done because these fields pull into OSHA reports and should not contain any PII.

Unfortunately, this update does not automatically update existing client systems and we ask that our support staff and clients update this manually and/or verify these updates have been made in your database already.

The 3 fields that need updated in the Incident Flowsheet are:

  1. What was the worker doing immediately prior to the injury/illness?
  2. How did the incident happen?
  3. Object or substance that directly caused the injury or illness?

The Incident Report flowsheet should be updated to ADD the text “Do not include any personally identifiable information (PII) (e.g. no names, phone numbers, SSNs) in this field.” in the Description column. Additionally, for the question “Object or substance that directly caused the injury or illness”, please REMOVE the text that says, “If exposure, please type source patient name”.

Lastly, when this information is added via the Incident Report on the portal, we pull 2 of the 3 questions into the encounter HPI. We also recommend adding this instructional text in the HPI flowsheet in the event additional context is added by the provider.

The 2 fields that need updated on the HPI flowsheet are:

  1. What was the worker doing immediately prior to the injury/illness?
  2. What happened? Tell us how this injury occurred?
